There is something odd with the partition table
Nov 2 16:21:59 ubuntu ubiquity: /dev/sda2: Created a new partition 2 of type 'Extended' and of size 128 GiB.
Nov 2 16:21:59 ubuntu ubiquity: /dev/sda3: Created a new partition 5 of type 'Linux swap / Solaris' and of size 2 GiB.
Nov 2 16:21:59 ubuntu ubiquity: /dev/sda6: Created a new partition 6 of type 'FreeBSD' and of size 2 GiB.
Nov 2 16:21:59 ubuntu ubiquity: /dev/sda7: Created a new partition 7 of type 'FreeBSD' and of size 124 GiB.

It should be 2,3,5 and 6 not 6 and 7. Hence the cannot resolve /dev/sda7
afterwards.
