Thank you for taking the time to report this bug and helping to make
Ubuntu better.

Ubuntu 18.10 reached EOL on July 18, 2019.  That closed the door to
upgrades from 18.04 LTS, so that was expected

Ubuntu 18.04 LTS has two supported & tested upgrade paths, one was to
18.10 (now closed), the other will be to Ubuntu 20.04 (once Ubuntu
20.04.1 has been released).

This installation looks like it was caused by a corrupted file system. I
notice in the logs

I've marked this bug as incomplete, and suggest you `fsck` (file system
check) your partition to fix errors, and try again. If you believe I'm
in error please leave a comment as to why, and you can return the status
to "New".
