It doesn't work for me, the relevant files mentioned here (e.g. /usr/share/initramfs-tools/hooks/cryptroot) have changed significantly in the meantime.
Only specifying header=</path> in crypttab does not work for me, the file is not included in the initramfs.

@therealmarv: Could you tell me how you got this to work? It seems the
systemd handling of the file ist not used by default in Debian/Ubuntu
systems!?

  Fix Released

Bug description:
  A detached luks header for a luks device is a new feature in
  cryptsetup 1.4.  This is a feature request to allow the unlocking of
  luks devices with detached headers, both as a root device (ie in the
  initrd) as well as using the init system.

  The attached patch only solves part of the issue, support in the init
  system.
