Ubuntu better. We noticed that you were installing the LTS version of
Ubuntu using an older version of the installation media.  The latest
point release of 16.04 is 16.04.3. Please try installing from that
version of the installation media and see if that resolves your issue.

  installer crashing

Status in ubiquity package in Ubuntu:
  Incomplete

Bug description:
  ubuntu 16.04 installation is failing at create user. Using a custom
  built image. When I change the below config I face this issue:

  /etc/pam.d/common-auth
  auth required pam_tally2.so onerr=fail audit silent deny=5 unlock_time=900

  /etc/pam.d/commom-passwd
  password sufficient pam_unix.so remember=5

  login.def 
  inactive maxdays mindays

  group file /etc
