Thank you for taking the time to report this issue and helping to make
Ubuntu better.

I would validate your media, as errors appear to be related to faulty
media, for example I see the following in your logs

I have marked this bug report 'Incomplete' as the problem appears to be
bad media to me, if however you believe I'm in error, please leave a
comment explaining why, and change the status back to "New".
