The log suggests that the disk is having error, more likely an
hardware/disk corruption problem than an ubiquity one...

Bug description:
  I have installed Ubuntu on a virgin SSD with an AMD Athlon, the OS
  bugged regularly, it was frozen then impossible to shut down. I also
  had lines of error messages

  I have made a second attempt for installation wich failed (see this
  related bug report. It seems to come from the disk.

  I have made those installations from a USB stick
