I think https://bugs.launchpad.net/ubuntu/+source/os-prober/+bug/1770173 might be related as well.
The installation hangs at the exact same point: 66% while running the "dmsetup create -r osprober-linux-something" command.

  The bug reported in https://bugs.launchpad.net/ubuntu/+source/os-
  prober/+bug/1663645 is still present in some cases and has been
  reported upstream as well: os-prober hangs when using LVM and
  cryptsetup.

  A fix has been released upstream: https://bugs.debian.org/cgi-
  bin/bugreport.cgi?bug=853927

  Could you please consider upgrading the ubuntu os-prober packages to
  1.75 or even 1.76?
