What hardware is this installation done on? Is it partitioned to do
RAID?

I see some errors that possibly indicate a hardware issue:
Oct 22 17:01:12 ubuntu grub-installer: info: Running chroot /target grub-install  --force "/dev/sdc"
Oct 22 17:01:12 ubuntu grub-installer: Installing for x86_64-efi platform.
Oct 22 17:02:59 ubuntu grub-installer: show_order(): Input/output error
Oct 22 17:02:59 ubuntu grub-installer: Could not delete variable: Input/output error

** Changed in: grub-installer (Ubuntu)
