Public bug reported:

During installation of a new 18.04.1 (ubuntu-18.04.1-live-server-
amd64.iso) the install would complete successfully, machine would reboot
and I would not be able to login. After dropping into a root shell in
recovery mode and looking at /etc/passwd, the user was not there.

I also noticed that the host name was not being properly set to the
assigned value during the install. Instead it was
"localhost.localdomain".

Moving to the alternate installer (ubuntu-18.04.1-server-amd64.iso) did
not have this issue.
