Your installation medium contains a mix of several different releases.
You need to format a USB stick before using it to install a different
release.

  Installer fails with Grub being the issue (16.2)

Status in grub-installer package in Ubuntu:
  Invalid

Bug description:

  Installing Ubuntu 16.0.2 on FX8370 with 8GB ram. Installing to 3rd
  hard drive {clean} 500GB HDD. Root filesystem is at the beging with
  3GB of swap allocated at the end.  Will not install grub corectly,
  causing the installer to fail every time.
