Your disk is currently partitioned to boot in bios mode, but you booted
the 16.04 installer in EFI mode. A warning message prompted you that
this could be a problem and recommended performing a bios mode install
instead, but you chose not to. You either need to perform the bios mode
install, or partition the disk using GPT and set up an EFI system
partition to install in EFI mode. This will be done for you if you
choose the "use entire disk" guided install option.

  Incomplete 16.4 install from DVD image

Status in grub-installer package in Ubuntu:
  Invalid

Bug description:
  Trying to upgrade to 16.4 version (previous one was probably 14.x).
  Internet update failed (on a black screen - had to reboot by hand).
  With an install disk the installation keeps sending bug reports (third
  try, the problem seems not to be at the same point). After reboot
  (thought there has been an error message) the system *seems* OK but I
  still have problems to read my DVD collection (which is why I keep on
  trying to reload the upgrade). I'll try again in a day or two...
