Your EFI system partition is corrupt.  You will need to either reformat
it, or try to repair it using dosfsck in linux, or chkdsk in Windows.

  Invalid

Bug description:
  I don't have a lot of additional information -- I'm trying to install
  Ubuntu 16 (latest stable or long term release) on an ext4 partition
  with Ubuntu 14 and windows 8 on other partitions and it crashes
  fatally when installing.
