I tried upgrading today using a virtual machine to which I applied your
apt-clone information attached to this bug report and I was able to
successfully upgrade to 16.04.  Could you please try the upgrade again
if you haven't sorted this out already?  Thanks in advance!

  1404 to 1604 upgrade - fails to update to new sources

Status in ubuntu-release-upgrader package in Ubuntu:
  Incomplete

Bug description:
  after running sudo update-manager -d and selecting upgrade and accept the updater.
  The updater gets to the second stage which is to update the sources and after a few seconds stops, gives the option to reprt a bug and then winds back the install.
