[Bug 1604441] [NEW] D-I on 16.04.1 breaks on usb probe and networking setup

Public bug reported:

On both netboot mini.iso and ubuntu server the installer fails probing
usb and then fails to setup networking, for netboot this means you can
not continue the install.

This is a critical fix required for 16.04.1
