Exactly the same problem on Ubuntu 16.04:

  update-manager does not update all available packages

Status in update-manager package in Ubuntu:
  Confirmed

Bug description:
  I recently observed that after updating the system using update
  manager a significant amount of packages remained pending and not
  upgraded.

  It can be verified on every update that is available:
  1. I get informed that updates are available or
     I start update-manager manually
  2. I get a list of packages available for update
     I install them or
  3. I call sudo apt-get upgrade (with or without previous update)
     => I get a list of packages pending (which in case 2 ..or are different and a superset of what update-manager offers)
  4. Manually upgrading works fine, upgrading using update-manager is unreliable
