Title:
During upgrade from 14.04 -> 16.04 the screen locked and I lost the
  ability to unlock

Status in ubuntu-release-upgrader package in Ubuntu:
  New

Bug description:
  1. Install a fresh 14.04.4 machine in a VM
  2. Update it
  3. Reboot, why not
  4. Run update-manager -d
  5. Click through and start the upgrade
  6. Check back on the VM periodically, notice the screen is locked
  7. Unlock the machine, see it's still got a way to go, come back later
  8. At some point (and I can't be more specific because I couldn't unlock the machine to see, I think it was near the end of downloading, so quite possibly it had started the upgrade - I came back to it, tried to unlock the screen and:
    8a Enter password
8b black screen for a second
    8c back to lock screen.

I will try and re-create this and get some more logs.
