It appears that both your NTFS and EFI system partitions are corrupt.
You will need to repair them from Windows.

Bug description:
Installed Ubuntu 14.04.3 on a fresh machine (dual-booting with windows
8.1). Rebooting after the install, and choosing Windows in grub, to
check that it still works. Rebooting again, and grub was not found
when booting up (defaulting to the windows bootloader).
Trying to reinstall Ubuntu (overwriting previous installation) then
fails to install grub, displaying an alert "This is a fatal error".
This happens on every subsequent install attempt.
