It looks like you reused the same usb stick to install 10.04 and 12.04,
so there are still leftover bits of 10.04 on the stick, corrupting the
install of 12.04. You need to format the stick clean before using usb
creator to make it bootable with 12.04.

Update 10.04 to 12.04 GRUB failed to install
Status in grub-installer package in Ubuntu:

Bug description:
Updating Ubuntu 10.04 to 12.04 from live DVD/CD. Error message showed,
stating GRUB failed to install, meaning it cannot launch ubuntu and
update aborted. Then gave me the option to subit crash error report,
which I did.
Expected to update but GRUB failed to install. Unable to check system
infromation.
