Thank you for taking the time to report this bug and helping to make
Ubuntu better. Reviewing your log files attached to this bug report it
seems that there is a problem with your installation media (CD/DVD or
USB stick). Please retry your installation with new media and if it
still fails I'd recommend testing your memory. In the event that is is
not in fact an error with your installation media or hardware please set
the bug's status back to New. Thanks and good luck!

Title:
  installing Ubuntu Trusty (14.04 lts) from DVD-drive in Virtualbox
  machine

Bug description:
  It very hard to attach the requested files. I am presented with a
  basic Firefox interface to report this crash/bug after the Virtualbox
  system said, or the installation said that the files could not be
  copied. The installation procedure suggested to clean the DVD player.
  The player is brand new and functioning normal up to now.

  I inserted the DVD with the Ubuntu Trusty iso 64bit burnt onto it in
  the systems DVD player and instructed the Virtualbox machine to use
  that player. As a disk I selected a virtual machine HD of 32GB on a
  256GB free hard drive.
