Additionally, https://launchpad.net/ubuntu/+archivemirrors should
monitor each of these addresses -- not just archive.canonical.com (which
is missing from the list, too) but each IP address it can resolve to.

The IP address 91.189.92.191 resolves to sadalbari.canonical.com and
indeed, http://sadalbari.canonical.com/dists/ does not contain the
trusty-updates and trusty-security directories (and trusty-proposed
looks fairly old, January 2014). I observed the same problem with
kudan.canonical.com earlier today.

Sorry for not being able to figure out which package to report this
problem against. I found something called "ubuntu-release-upgrader"
but I do not have a package with that name.
What I expect to happen:
(a) If a mirror is incomplete or otherwise inconvenient, there should be an option to choose a different mirror
(b) kudan.canonical.com and sadalbari.canonical.com need to be synced so that they offer the correct files and do not prevent upgrades until (a) is fixed.
What actually happens:
The upgrade is stopped, and changed settings are changed back to
12.04.
