[Bug 1383295] [NEW] Installing into pre-existing lvm/luks partition succeeds, but system does not boot

Public bug reported:

Installing Ubuntu 14.04 .1 LTS on pre-existing LVM encrypted with LUKS
fails to create file /etc/crypttab, thus system is not bootable.

My report:
I had Fedora installation with LVM/LUKS and separate boot partition. I created new volume for ubuntu, then using Unity unlocked the partition. When asked where to install I chose 'Something else' and defined mount points for /boot, / (new LVM volume), /home (existing LVM volume). Everything installed without error, but after reboot I was greeted with initramfs console, and a message telling me / was not mounted. After I created /etc/crypttab and recreated initramfs image I was asked for password and the issue was fixed.
