  duplicate source.list entries error during installation

Status in “ubiquity” package in Ubuntu:
  Incomplete

Bug description:
  Near the end of installation, installer crashed.  Log viewable during
  installation showed errors about duplicate source.list entries.
  Actual package is "ubuntu-12.04.3-desktop-amd64.iso", but was not
  available for choice.
