Your installation medium contains packages leftover from debian that
conflict with ubuntu packages.  You need to format the usb stick before
using usb creator to set it up.

Status in ubiquity package in Ubuntu:
  Invalid

Bug description:
  Sorry, not sure what happened.  Was installing in place of Debian 7.7
  with the install wizard; chosing to wipe drive and install LVM.  Will
  try again by wiping the drive first.  May be issues with hardware;
  older machine.  Hope this helps.  Will try to attach syslog and
  partman logs.
