We will need a copy of /var/log/syslog to see what happened. Also
please run the check cd option from the boot menu.

Status in “ubiquity” package in Ubuntu:
  Incomplete

Bug description:
The installer encountered an error copying files to the hard disk...

  [Erno 5] Input /output error

This is often due to faulty CD/DVD disk drive, or a faulty hard disk. It may help to clean to the CD/DVD, to burn the CD/DVD at a lower speed , to clean the CD/DVD drive leans...
  We're sorry installer crashed...

I try the installation on several devices (Old devices like HP compag nx-6110-Intel celeron m, AMD Athlon 3600+)... with different CD's from different CD drives...
The installation stop's after the 70 - 80% of copying files!

  Ubuntu 121.04LTS
