Your installation medium is a mishmash of the different releases you
have put on it.  Please format it before reusing it for a different
release.

  GRUB failed to install after re-installing 12.04 over a non-working

Status in “grub-installer” package in Ubuntu:
  Invalid

Bug description:
  My new Samsung series 5 used to run 12.04 beautifully. I upgraded to
  12.10 and got the black screen after the Ubuntu splash page. So, I
  installed 13.04 over 12.10. This didn't work either, so I re-installed
  12.04 over 13.04. The GRUB installer at the very end of the
  installation process failed to work and the whole thing is a wash
  now... fml
