FYI, even without the update options selected, I still got the same
error. I'ma try falling back to 12.04.0 and doing an upgrade.

  installer just crashed on grub. i had "install updates" and "use
  restricted software" selected, but aside from that, this was a 100%
  default install. i'm going to retry without those options checked now.
