Title:
Failure to mount root filesystem, cause unknown

Status in “initramfs-tools” package in Ubuntu:
  Invalid

Bug description:
This report and its comments describe symptoms of a problem wherein
the root filesystem is not mounted after a kernel upgrade.

It is unlikely that they share a common cause, and further analysis is
  needed to isolate the problem(s).
