For us to be able to investigate this matter it would really help to
have log files from the failing installation.  You can find them in
/var/log/installer/ on an installed system or by grabbing
/var/log/syslog from the system that is failing to install.

  Warning about a faulty disk or DVD when installing Ubuntu

Status in “ubiquity” package in Ubuntu:
  Incomplete

Bug description:
  when installing 13.04 or 12.10 from their installation media's (tried
  USB DVD and RW) and they al shows this message sometimes. it happand
  mee two times in a virtualbox and other physical machines, and six
  times no my Clevo Inc. Xnotebook turbine(w370ET). but when i
  completely remove all the partitions and let ubuntu create them
  automatically, it 'Sometimes' works. but not every time.

  the message everytime keep telling me that i can buy cleanup products
  for my DVD in most electronic stores.

  dont know what the problem triggers, my ex scoolmates also had this
  issue when installing on a VM. this message shows up half in the
  installation process. when copying files when 40% / 60% done.

  i had a thinking, it might be the problem when having too much
  partitions. because installing ubuntu on an empty disk, doesnt trigger
  this error.
