I was using an ISO image in ESXi. I'll re-download the image file and try it again.

In the syslog log file we see the following:

Given that this package appears on the CD it seems likely that there is
an issue with the disc from which you were installing Ubuntu. I'd try
creating one again and installing and see if that resolves the issue.
