The initramfs-tools change that makes this work in raring is:

initramfs-tools (0.103ubuntu0.3) raring; urgency=low

  * Add nfsv3, nfsv4 and nfs_acl to the base modules.
    This should fix nfs from initramfs.

I think just those modules are necessary and not nfs_layout_nfsv41_files
(or maybe it will be pulled in automatically).

I just marked my bug #1217041 as a duplicate of this one, but please
consider uploading the debdiff I posted there.

Bug description:
  Unable to mount nfs correctly with PXE install server, which works no
  issue with 12.04.2 and 13.04.

  Encountered this issue with 12.04.3 destkop CD image[0], both amd64
  and i386 image have the same problem.
