I just found this bug today and I had the same issue here in ubuntu
12.04 with HP Pavilion dv6

Bug description:
Binary package hint: udisks
This is using an internal card reader on a Pavilion DV7. When
inserting an SD Card the first time, it mounts correctly. However,
after unmounting and reinserting the system can not recognize it.
Rebooting solves the problem. There is no message in dmesg when
reconnecting.
