Title:
  Installation crashed!

Status in “ubiquity” package in Ubuntu:
  New

Bug description:
I installed Ubuntu 13.04 three months before; but the graphical response was very very slow if compared with the previous versions of Ubuntu that I used. Then I added an nVidia GeForce GT610 graphics card and the response was better though the boot screen got in to fallback mode (low resolution purple screen)!
Thereafter, problem starts with updating karnel (automatic updates) and a specific update of kernel named "base" failed repeatedly. So I could not install regular updated. Then the screen got freezed randomly and the machine needed a hard reboot.

Last issue faced was with unable to mount hd0 drive and the GRUB did
  not show up!

I, then decided to go for a fresh installation and the installation
  failed at 99% progrss bar!
