Thank you for taking the time to report this bug and helping to make Ubuntu better.  I noticed that you are installing an older version of Ubuntu, unless you have some specific requirements to run an older release I recommend trying to install the latest version of Ubuntu.  We released version 12.04, in April of 2012, and it is a Long Term Support release.  You can find different ways to obtain it at http://download.ubuntu.com.  In the event that you really want to run the older version of Ubuntu please set this bug's status back to New.  Thanks!

  ubuntu 11.04 installer crashes trying to format harddrive

Status in “ubiquity” package in Ubuntu:
  Incomplete

Bug description:
  the message it had at the bottom of the installer was "Creating ext4
  file system for / in partition #1 of SCSI3 (0,0,0) (sda)..."

  and the message in the popup box said to use this weblink to submit a
  new bug report (not add to old one), and attach /var/log/syslog and
  /var/log/partman, but i couldnt attach both files, so i cat'd them
  onto a single file and attached that.
