Title:
  Upgrade from 10.04 LTS to 12.04.1 on i386 Ubuntu (headless) Server
  fails

Status in “initramfs-tools” package in Ubuntu:
  New

Bug description:
  package initramfs-tools 0.99ubuntu13 failed to install/upgrade:
  ErrorMessage: subprocess installed post-installation script returned
  error exit status 1

  'do-release-upgrade' reported an error and tried to launch a browser
  using "xdg-open" but failed (presumably since it would have to use a
  text-only user agent to file the bug.

  Failure is due to /boot/ file-system hitting 100%

  df
  Filesystem 1K-blocks Used Available Use% Mounted on
  /dev/sda5                                      233335  222523         0 100% /boot

  This is the same as bug #798414 but should be handled properly, not just giving a warning. This should be part of the pre-upgrade checks that ensure sufficient space.
