Sound output level is ridiculously high

I have many, many problems with sound on Karmic. Usually only one app
can play sound properly - it "steals" sound from the other apps. At
the moment it's Steam.exe running on wine.
At the moment I can play Steam games and sound works fine. However,
look at the sound output level - it's obviously wrong.
I'm on 64-bit Karmic alpha 4 with a few updates since alpha 4 but not
all of them. Sound support on my second-gen MacBook has actually got
worse during the Karmic development cycle. Jaunty never gave me a
problem with sound.
