May be it is a software problem, but when reinstalling shows similiar
errors you should think about a damaged harddrive. Only a possibility,
you know.
May be a testinstallation with a newer kernel(because of ext4 errors)
may be helpfull to determine what it is.
If you are willing to do so here is the link to the upcoming Ubuntu12.10
Quantal which incorporates the 3.5.x kernel:
