Bug description:
  I've seen this a few times, most recently on 20121005 armhf omap4
  desktop image.  This is reproducible frequently, but doesn't happen
  every time.

  When booting the install from sd to install, occasionally my keyboard
  and mouse do not work.  I'm using a logitech combination
  keyboard/mouse.  If I unplug the usb connection and plug it back in,
  it will usually come back and start working right after that.  I
  talked to ogra a bit about this and he mentioned that it could be a
  udev race, so I'm starting here.
