There are several different methods listed on this web page.  Which
method did you use?

The preferred method, usb-creator, should not have given you a fat
filesystem.  So you've wound up with a corrupted image here.

  Using the Alternate CD to do an upgrade to 12.04 and not used the
  network there are missing files:

  Failed to fetch cdrom:[Kubuntu 12.04 LTS _Precise Pangolin_ - Release i386 (20120424)]/pool/main/x/xserver-xorg-video-ati/xserver-xorg-video-ati_6.14.99~git20111219.aacbd629-0ubuntu2_i386.deb File not found

  A quick find shows differently named files in the image:
