This may be an issue with AMD cpus, I've seen a similar problem with eglibc: bug 956051
I can confirm that using the Ubuntu 12.04 LTS 64bit minimal CD to do a net install, it gets past the initial cdrom problem, but then comes up with the dpkg error:
Warning: Failure trying to run: chroot /target dpkg --force-depends --install /var/cache/apt/archives/debconf_1.5.42ubuntu1_all.deb

In the log console while the installer is running (ctrl-alt-f4) I see
the following (which leads me to believe it's the above bug):

kernel: [ 2051.425647] pycompile[20389] trap invalid opcode ip:7f84167cae11 sp:7fffc8ca98e0 error:0 in libm-2.15.so[7f8416788000+f9000]
deboostrap: Illegal instruction
debootstrap: dpkg: error processing debconf (--install):

Using: XenServer 6.0.201
server: 2xAMD 12 core 6238 cpus

Title:
Regression: Error - Ubuntu-server 12.04 installation as VM on xen-
server 6 (use of local iso file)

Status in “debian-installer” package in Ubuntu:
  Incomplete

Bug description:
It is *not* possible anymore to use a local ubuntu-server-12.04.iso file for off-line installation of VM on XCP 1.5 (xen-server 6).
The created VM boots first correctly from the iso file and allows keyboard settings etc *but* somewhere later it breakes with the following error message:
  # ---
Your installation CD-ROM couldn't be mounted. This probably means that the CD-ROM was not in the drive. If so you can instert it and try again.

  Retry mounting the CD-ROM?
  # ---

This ubuntu-server 12.04 image can be installed on bare metal (HP DL385g7) without any troubles.
Also ubuntu sever 11.10 iso image can be successfully installed as VM from a local iso file on XCP 1.5.
I have repeated the test few times and on two different (fresh insalled) XCP hosts.

Both iso images are saved localy on the XCP host in the following directories:
  /var/opt/xen/iso_import/ubuntu11.10/
  /var/opt/xen/iso_import/ubuntu12.04/

What has changed in 12.04 compare to 11.10 due to the mounting of CD-
ROM during the installation? (the CD-ROM label has additional "LTS"
text for 12.04). It is curious as the image boots in firts phase and
is "inserted" into the virtual CD drive of XCP 1.5.

  regards, Mark
