Title:
  cryptsetup fails to create swap

Status in “cryptsetup” package in Ubuntu:
  Expired

Bug description:
I created an encrypted swap partition (as I've done many times), but
cryptsetup, when called from /etc/init.d/cryptdisks-early fails and
  stalls.

The only way to boot is to press ctrl-alt-dell at that point and it
resumes booting (without adding the swap partition).

I added a "set -x" to check where the boot process stalls and it does
  after issuing:

  cryptsetup -c aes-cbc-essiv:sha256 -s 256 -h sha256 --key-
  file/dev/urandom create cryptoswap /dev/sda4

I tried to issue the command manually and it simply says:
  "command failed"

I disabled the encrypted swap on my other machine, removed the device
from device mapper, run the same command and it run OK.

I don't know why it's not working on my notebook...
