  installation crash after log/sign-in on / formatted partition

Status in “ubiquity” package in Ubuntu:
  New

Bug description:
  After installing Ubuntu 12.04 ( / and /home partitions created during
  install) using the /home I went back to install on / partition and of
  course no / or / home so I put /home back on where I installed OS and
  reformatted / and installed to the point where I was (who knows where
  the system install was) entering name and password.  Kerplunk and here
  I am.  The first install did not have a list of "media", etc. in the
  "Finder" (the first icon on desktop) so all I had was the first line
  of recent and could not know what I have installed (packages) if it
  did not have an icon.  I do not know if they are related but that was
  why I was installing again on the second partition.  I decided to keep
  the "files" it said I no longer needed (after first install finished)
  so I could try and someday have OpenOffice, maybe, and that may be why
  I am stuck with no list (menu) of packages and writing my first bug
  report.  oh, yeah, I have successfully installed 12.04 on this
  computer before and that is how I saw the OpenOffice files being
  removed.  But these two installs and the one before these two, also
  today, (it kept popping up "system problem" or something like that)
  were different somehow though I do not know how and I do not know how
  to attach the files /var/log/syslog and /var/log/partman (it was
  during install.)  My computer is a 2009 Compaq CQ 5700F with AMD
  Athlon II 170u processor and AMD 3000 graphics card (256K) and 2 GB
  RAM.  I had Kubuntu on it and it was fast (rekong) and efficient and
  effective.  It was running awesome!  I also did the "ufw enable" and
  "ufw logging on" thru the terminal and maybe that is what broke these
  installs.
