Thanks so much Maxime.  Indeed it seems that there is no bug here.  I
didn't see anything in the change logs for the debian-installer package
it self.  But lo and behold, based on your tip, I found that it was
indeed changed in the Installation Guide docs.

Status in “debian-installer” package in Ubuntu:
  Invalid

Bug description:
  From my testing so far this problem began with oneiric and continues
  with precise.  Lucid and Natty work as expected.

  What I expect to happen:

  1) boot mini, alternate, or server install ISO (i.e. any debian-installer based ISO)
  2) append "netcfg/disable_dhcp=true" before the double-dash in the kernel command line
  3) start installer
  4) be prompted for IP configuration

  
  What happens:

  1) boot mini, alternate, or server install ISO
  2) append "netcfg/disable_dhcp=true" before the double-dash in the kernel command line
  3) start installer
  4) d-i attempts to configure with DHCP
