seems that there is a problem with the disk to which you are trying to
install Ubuntu.  I'd recommend performing a back up of your existing
operating system and then investigating the situation.  Measures you
might take include checking cable connections for your disks and using
software tools to investigate the health of your hardware.  In the event
that is is not in fact an error with your hardware please set the bug's

  11.10 crashed during install

Status in “ubiquity” package in Ubuntu:
  Invalid

Bug description:
  I'm trying to install 11.10 over 10.04. I don't know why it crashed
  but it's letting me do this bug report. The files that you require to
  read I believe the installer sent them just before it gave me access
  to the internet to finish the report. I have no access to the file
  system at present and have no idea how this is actually working right
  now.
