Title:
Upgrade menu option should not appear for old releases

Bug description:
As documented in bug 891711 if you insert a bootable disk with a
previous release of Ubuntu, say 11.10, into a system running a later
release of Ubuntu (12.04) you are presented with an option to
"Upgrade" from the new release to the old one. This reinstalls Ubuntu
while preserving your home and installed applications. However, this
was not an envisioned use case (and subsequently is not tested) and is
problematic as the kernel from the new release is not removed.

The option to upgrade from a new release to an old release should not
  appear in Ubiquity.
