[Bug 930624] [NEW] add source dialog problems

Public bug reported:

The Add source dialog accessible from the Other Software tab's Add... button has two problems:
- it does not have a title set, thus the title is software-properties-gtk, and should be "Add" (based on the HIG, the title should be the label of the action opening the dialog without the trailing ellipsis)
- it shows the add icon regardless of the buttons-have-icons system-wide setting (it should respect system-wide settings)
