I believe this should me marked as solved. The issue was caused by a
flaky DVD drive. I noticed it was sometimes not showing in BIOS. Also,
the machine being dual-boot, I was in Windows 7, looked at Device
Manager and noticed an 'X' next to the aforementioned DVD drive.
Disconnecting the drive from the motherboard resolved the issue.

Title:
  Installer Crashed

Status in “ubiquity” package in Ubuntu:
  Incomplete

Bug description:
I tried to reinstall Ubuntu 11.04 64-bit AMD from CD, which I burned.
The installer crashed, giving me a note to file a bug report here.

I was past the partitioning phase and the installer said it was
removing conflicting files (no doubt from my previous install which
  was borked).

I did not tell the installer to format any of the partitions; perhaps
this is the cause of the problem.

I have combined the requested log files into one attachment, which is
  all that is allowed here.
