[Bug 1091556] [NEW] dm-crypt passphrase screen shows blank screen 25% of boots

Public bug reported:

Roughly 25% of the times I boot my computer, I get a completely blank
screen instead of the dm-crypt passphrase prompt. Typing in the
passphrase works buts results in a blank screen when logging into ubuntu
(or kubuntu in my case). This could be resolved by pulling the monitor
plug and plugging it back in again (or probably by switching screens
with CTRL-ALT-F6 /F7 as OP noted).

Computer is a Dell laptop with a monitor plugged in via HDMI on boot.
Unsure exactly which dumps are useful:
