Removing old linux images etc freed up sufficient disk space for initramfs-tools to be re-installed with Synaptic. Problem caused by insufficient disk space on /boot partition during upgrade. This in turn was  caused by allowing the number of available linux images to build up during updates. 
It would be useful if a disk space check were performed prior to upgrade.

  Upgrade from 10.11 to 11.04. Upgrade reported low disk although the
  error message contained no legible text as each character was a
  square. Message box was closed using the "Close" icon on the window
  bar and the upgrade continued.
