I am also facing this problem. I cannot even add an image with
"Other..." button. No errors, no messages while launched in console.

Binary package hint: usb-creator
On Karmic.
I can't add any iso file to the menu nor dragging it with the mouse nor choosing it through the menu.
If run from the command-line:
streinsacu at roncalle:~$ usb-creator-gtk --ISO=~/Downloads/ubuntu-9.10-alternate-i386.iso
Usage: usb-creator-gtk [options]
usb-creator-gtk: error: no such option: --ISO
streinsacu at roncalle:~$ usb-creator-gtk --iso=~/Downloads/ubuntu-9.10-alternate-i386.iso
