[Bug 52800] New account wizard only displays username (exchange)

Public bug reported:

when trying to set up a new account and going through the wizard, if I
choose "Server type: microsoft exchange" then the only field I am given
to fill in is Username.  If I fill this in, I cannot choose "Forward".
